DESCRIPTION:At any time in your life\, you may be unable to communicate your health care preferences. Advance care planning encourages the understanding of possible healthcare choices\, talking about decisions with loved ones and doctors\, and putting plans in writing if needed. It is a gift you can give to yourself and those you love. Having a plan will make it easier for you\, your doctor\, and your loved ones to ensure your wishes are honored.\n\n\n\nThis free workshop will help identify health care choices and suggest ways to begin communicating them to family and health care providers.It will also explain the use of Advance Directive documents - Living Will and Health Care Power of Attorney - to legally express your choices.\n\n\n\nForms\, resources\, and notary services will be provided for those wishing to execute their advance care planning documents.\n\n\n\nQuestions or RSVPs to 910-523-9792 or jennifer.jordan@lifecare.org\n\n\n\nPresented by Lower Cape Fear Hospice\, a nonprofit organization dedicated to providing the highest level of care and comfort to patients with life-limiting illness\; support & counseling to families\; and education to the community.
